INSTRUCTIONS:
1.        Print this application and fill it out.  It  must be typed, or printed neatly.
2.        Additional pages may be attached to complete any section.
3.        Include one letter of recommendation from a previous teacher at the University level  with a daytime phone number. 
4.        The essay portion must be typed, double spaced, on separate sheet(s).
5.        The completed application must be submitted to the address on the application no later than November 1, 2002.
6.        The (3) finalists will be interviewed at the University of Houston by the selection panel.
7.        There will be one $1,000.00 scholarship awarded in 2002, a check will be made payable directly to the University of Houston.
8.      Scholarship winner will be notified on Friday November 22, 2002.

ELIGIBILITY:  Applicants shall be full time, junior, senior or graduate students at the University of Houston whom are a current member of the University of Houston Student Chapter of the IESNA and desire to incorporate illumination into their future career.   Click  for Membership Application
ESSAY:  Please include a typed, double spaced essay, 500 words in length, illustrating what lighting means to you, how you plan to incorporate it into your career and how you would benefit from receiving the scholarship.
LETTER OF RECOMMENDATION:  Please include one letter of recommendation from a teacher at the University level including their daytime phone number. 
DEADLINE: Return your completed application and essay no later than November 1, 2002 to:  Karen Gregory / Reliant Energy / 1301 Travis  Suite 1635 / Houston, TX  77002.
OBLIGATIONS:  Scholarship recipient has no obligation to the IESNA San Jacinto Section other than to attend the formal scholarship presentation at the IIDA Awards dinner on February 11, 2003 at the Plaza Club as our guest.  If not a current Member, you must also fill out a membership application, submit it on-line, print a copy and include it along with a $15 check made out to IESNA, and include it with your scholarship application
